Please fill out all post-observation activities below.​

This form should not be used in place of an immediate call to the Salem Police for an illegal incident.

​

This form is not an official police report.

To make a police report, a homeowner must contact 9-1-1 or call the Salem Police non-emergency line
at 503-588-6123.

Please note: The BCC Office will not share the homeowner's contact information with anyone.

​

Photos/Video
If it's possible and safe, snap a photo or take a video of anything that would assist with identifying the activities. Please email photos and videos to the BCC office at: battlecreekcommons@gmail.com
